صفحات 5-8مقدمهصرع میوکلونیک جوانان یک سندرم صرعی منتشرشونده می باشد که سن شروع آن عمدتا 12 تا 18 سالگی می باشد. این سندرم از لحاظ بالینی از پرش های میوکلونیک، تشنج های تونیک - کلونیک منتشرشونده و تشنج های غیابی تیپیک تشکیل گردیده است. تشخیص این سندرم در برخی موارد با دشواری رو به رو شده و با اشتباه در تشخیص و تجویز داروهای نامناسب احتمال تشدید حملات وجود دارد. با استفاده از VEM می توان به تشخیص دقیق موارد مبهم کمک شایانی نمود. هدف این مطالعه ارزیابی نقش تشخیصی VEM در صرع های مقاوم به درمان بوده است.مواد و روش هااین پژوهش به روش گذشته نگر و توصیفی بر روی بیمارانی که طی سال 1390 در بخش مغز و اعصاب بیمارستان رضوی مشهد با تشخیص صرع مقاوم به درمان جهت VEM بستری شده بودند انجام گردید. از بیماران جهت ارزیابی نتایج درمان مصاحبه تلفنی در فاصله زمان 18- 6 ماه بعد از ترخیص انجام شد.یافته هااز میان 250 بیمار که با سابقه ابتلا به صرع مقاوم به درمان بستری شده بودند، در مجموع 24 بیمار که با تشخیص نهایی صرع میوکلونیک جوانان ترخیص شده بودند، مورد مطالعه قرارگرفتند. 14 بیمار زن بوده و سن متوسط بیماران 24 سال و سن متوسط شروع بیماری 97/12 سالگی بود. حملات GTC قبل از انجام VEM، تعداد 76/2 حمله در ماه بود که بعد از انجام این تست و اصلاح درمان به 27/0 حمله در ماه رسید که کاهش معنی داری را نشان داد. میزان پلی تراپی نیز بطور معنی دار در بیماران بعد از انجام VEM کاهش یافت.نتیجه گیریبا انجام VEM در بیماران مبتلا به حملات کنترل نشده می توان مواردی را که بطور کاذب دچار صرع مقاوم به درمان شده اند تشخیص داد و با اصلاح برنامه درمانی، کاهش محسوسی در تعداد حملات آنها ایجاد کرد.کلیدواژگان: تشنج، صرع، صرع میوکلونیک جوانان، بیمار

صفحات 9-16مقدمهویژگی های منحصر به فرد سلول های بنیادی رویانی نظیر تکثیر نامتناهی و تمایز به انواع سلول ها آن را به ابزار مناسبی برای تحقیقات زیست پزشکی در زمینه هایی مانند کاربرد درمانی برای بیماری های تحلیل برنده عصبی نظیر پارکینسون و آلزایمر تبدیل نموده است. در سال های اخیر، روش های آزمایشگاهی زیادی توسعه یافته اند که اجازه تولید نورون از سلول های پرتوان را در کشت فراهم می کند. این سلول ها اگر بر روی لایه تغذیه کننده یا در حضور فاکتور مهارکننده لوسمیLIF تکثیر یابند حالت تمایز نیافته خود را حفظ می نمایند. اسیدرتینوئیک یکی از مهم ترین مورفوژن ها است و توزیع آن در مرحله جنینی با تمایز نورون ها و ویژگی مکانی آن ها در سیستم عصبی مرکزی در حال تکوین مرتبط می باشد.مواد و روش هابرای تمایز آزمایشگاهی در این پژوهش، دودمان سلولی CCE پس از تکثیر بر روی لایه تغذیه کننده فیبروبلاست جنینی موش و در حضور LIF، برای تولید تجمعات سلولی (اجسام شبه جنینی) کشت داده شد. سپس این اجسام شبه جنینی طبق پروتکل +4/-4 (چهار روز در حضور و عدم حضور رتینوئیک اسید) در معرض اسید رتینوئیک با غلطت 6- 10 مولار قرار گرفت. سپس بررسی های مورفولوژیک، ایمنوسیتوشیمی و مولکولی برای ارزیابی فاکتورهای عصبی صورت پذیرفت.یافته هادر این پروتکل القایی درصد بالایی از سلول ها (80%) به سلول های شبه عصبی تمایز پیدا کردند. این سلول ها، نشانگر سلول های نورواپی تلیالی یعنی نستین را بیان نمودند. همچنین نتایج نشان داد که اسید رتینوئیک می تواند بیان ژن فاکتور رشد عصب NGF را در سلول های بنیادی رویانی القا کند.نتیجه گیریاین یافته ها نشان می دهد که اسید رتینوئیک به شدت تشکیل و ویژگی نورون ها را در طول تمایز سلول های بنیادی جنینی موش تنظیم می کند و می تواند یکی از فاکتورهای موثر جهت تمایز این سلول ها به سلول های عصبی باشد.کلیدواژگان: سلول های بنیادی جنینی، اجسام شبه جنینی، نستین، نورون

صفحات 17-21مقدمهمصدومیت ها شایع ترین علت مرگ در سنین 1 تا 34 سال را تشکیل داده و مهمترین عامل ناتوانی به شمار می رود. حوادث ترافیکی یکی از علل مرگ و ناتوانی به علت مصدومیت می باشد که در طی آن سر و گردن به علت حرکت نوسانی، در معرض آسیب قرار می گیرند. این مطالعه الگوی صدمات سر و گردن در مصدومین بستری شده در بیمارستان های تهران را به تصویر می کشد.مواد و روش هادر این مطالعه مقطعی تمام مصدومینی که طی سال 1999 در 6 بیمارستان اصلی تهران بستری شده بودند، مورد بررسی قرار گرفتند. 2807 نفر از ایشان که دچار مصدومیت در ناحیه سر و گردن شده بودند، واجد شرایط ورود به این مطالعه بودند. اطلاعات این دسته از مصدومین توسط پزشکانی که درگیر درمان بیماران نبودند، صرفا به منظور پژوهش گردآوری شد. به منظور کنترل کیفی، اطلاعات نمونه ای تصادفی از این بیماران به طور مستمر مورد بازبینی و کنترل قرار می گرفت.یافته هامصدومیت سر و گردن در یک سوم مصدومین بستری شده در بیمارستان های مورد مطالعه مشاهده شد. در بین این دسته از مصدومین، اکثریت با مردان بوده و سن 75% ایشان 40 سال یا کمتر بود. شایع ترین علت مصدومیت سر و گردن، حوادث ترافیکی و پس از آن، سقوط بود.نتیجه گیریارتقای ایمنی به منظور جلوگیری از حوادث ترافیکی و سقوط، برای کاهش مصدومیت های سر و گردن ضروری است.کلیدواژگان: سر، گردن، زخم ها و جراحات، اپیدمیولوژی، ایران

صفحات 22-28مقدمهاصطلاح مهار منتشر شونده برای توصیف موجی استفاده می شود که پس از آغاز در یک ناحیه از مغز در سرتاسر آن گسترش یافته و اختلالات یونی و تورم سلولی را به همراه دارد. آزاد شدن گلوتامات در فضای خارج سلولی که به دنبال دپلاریزاسیون سلول ها اتفاق می افتد، یکی از مهمترین وقایع این پدیده است. هیپوکمپ در برخی از اعمال سیستم عصبی از جمله حافظه نقش دارد. هدف از این مطالعه بررسی نقش گیرنده گلوتاماتی NMDA در اختلال حافظه ناشی از مهار منتشر شونده در موش صحرایی بود.مواد و روش هادر بررسی حاضر 36 سر موش صحرایی نابالغ نژاد ویستار مورد بررسی قرار گرفتند. نقش القای مکرر مهار منتشر شونده بر حافظه و همین طور تاثیر مهار گیرنده های NMDA توسط MK-801 در آزمون رفتاری به وسیله T-maze در طول چهار هفته متوالی پس از القای مهار منتشر شونده مورد بررسی قرار گرفت.یافته هابررسی به دنبال القای مکرر مهار منتشر شونده نشان داد که این پدیده می تواند موجب وارد آمدن آسیب به حافظه گردد. در حالی که استفاده از MK-801 توانست بطور معناداری از شدت آسیب وارده به حافظه جلوگیری کند.نتیجه گیرییافته های ما پیشنهاد می کند که گیرنده های NMDA ممکن است یک نقش کلیدی در محافظت از حافظه در اختلالات مرتبط با مهار منتشر شونده ایفا کند.کلیدواژگان: مهار منتشر شونده قشری، هیپوکمپ، حافظه، مغز

صفحات 29-33مقدمهاستفاده از پیش دارو قبل از عمل جراحی و با تاثیرات تسکین درد منجر به کاهش حساس شدن و یا مسدود شدن مسیر انتقال درد خواهد شد. از جمله این داروها تیزانیدین و گاباپنتین می باشد. تیزانیدین در مقایسه با گاباپنتین دارای عوارض جانبی بیشتر و طول عمر کوتاهتری می باشد و مطالعات محدودی در رابطه با اثر گاباپنتین به عنوان پیش دارو بر درد بعد از عمل وجود دارد. در این مطالعه به بررسی و مقایسه اثر گاباپنتین خوراکی بر درد پس از عمل جراحی هیسترکتومی الکتیو با تیزانیدین خوراکی پرداخته شده است.مواد و روش هامطالعه ی حاضر بر روی 64 بیمار30 تا 60 ساله کاندید عمل جراحی هیسترکتومی الکتیو در بیمارستان شریعتی انجام شد. بیماران به صورت تصادفی به دو گروه 32نفره تقسیم شدند. گروه 1(گاباپنتین) که یک ساعت قبل از عمل 300 میلی گرم گاباپنتین خوراکی و گروه 2 (تیزانیدین) که یک ساعت قبل از عمل 8 میلی گرم تیزانیدین خوراکی دریافت کردند. درد قبل از تجویز این داروها و در 12 ساعت اول پس از عمل با استفاده از visual analogue scales (VAS) ثبت شد. همچنین میزان مورفین مصرفی در 12 ساعت اول و اولین زمان در خواست مورفین از طرف بیمار ثبت و با یکدیگر مقایسه شد.یافته هامیانگین سنی در دو گروه تفاوت معناداری با یکدیگر نداشت. میزان ماده ی مخدر مصرفی در گروه یک کاهش معناداری نسبت به گروه دو نشان داد. همچنین اولین زمان درخواست مخدر در گروه یک نسبت به گروه دو افزایش معناداری نشان داد. درد قبل از عمل جراحی میان دو گروه اختلاف آماری معناداری با یکدیگر نداشت، اما 1 ساعت ، 3 ساعت و 12 ساعت پس از عمل در گروه یک کاهش معناداری نسبت به گروه دو نشان داد.نتیجه گیریبر اساس نتایج مطالعه، گاباپنتین خوراکی پیش از عمل نسبت به تیزانیدین خوراکی قبل عمل سبب کاهش درد و کاهش نیاز به مخدر در حین و پس از عمل جراحی می شود.کلیدواژگان: تیزانیدین، گاباپنتین، هیسترکتومی

صفحات 34-41مقدمهتغییر دادن عملکرد سلول های عصبی به وسیله تحریکات الکتریکی و عوامل دارویی در پژوهش های علوم اعصاب به منظور کاوش در چگونگی کارکرد مدارهای عصبی، رفتارهای مربوط به این مدارها و درمان بیماری های عصبی از موضوعات مورد توجه و علاقه پژوهشگران بوده است. قدرت روش های الکتروفیزیولوژی و فارماکولوژی در میزان دقت تحریکات سلول های عصبی از لحاظ زمانی و فضایی متفاوت است و در دو دهه اخیر دانشمندان سعی در بالا بردن هر کدام از این مزیت ها داشته اند. قدرت روش های اوپتوژنتیک در بدست گرفتن هم زمان کنترل زمانی و فضایی در تحریک سلول های عصبی است. در اوپتوژنتیک، پژوهشگر نور را به سلول های مورد نظر که با مولکول های حساس به نور همراه هستند می تاباند و عملکرد سلول های عصبی را تحت کنترل در می آورد.نتیجه گیریاخیرا از اوپتوژنتیک برای شناخت چگونگی کارکرد سلول ها، مدارها و سیستم های عصبی استفاده شده است. علاوه بر این، محققین در حال تلاش در جهت شناخت و درمان بیماری های سیستم عصبی با استفاده از اوپتوژنتیک هستند. در این مقاله مروری، به معرفی این روش نوین و کاربردهای درمانی احتمالی آن اشاره خواهد شد.کلیدواژگان: الکتروفیزیولوژی، اوپتوژنتیک، اوپسین

صفحات 42-49مقدمهگیرنده وانیلوئیدی نوع 1 متعلق به خانواده کانالهای یونی وابسته به لیگاند است. این کانال کاتیونی غیر انتخابی به سدیم، پتاسیم و مخصوصا به کلسیم نفوذپذیری دارد. ورود کلسیم از طریق آن منجر به فعال شدن سیگنالینگ داخل سلولی می شود. هر چند که این رسپتور فقط در شاخ خلفی نخاع وجود دارد اما مطالعات بیشتر نشان دهنده حضور فعال آن در سایر مناطق سیستم عصبی مرکزی و مغز می باشد.نتیجه گیریشواهدی وجود دارد که نشان می دهد که گیرنده های TRPV1 در مغز، در بسیاری از عملکردهای اساسی عصبی از جمله انتقال سیناپسی و شکل پذیری سیناپسی نقش دارند.کلیدواژگان: گیرنده TRPV1، انتقال سیناپسی، شکل پذیری نورونی

صفحات 50-54مقدمهبیماری میگرن یکی از شایعترین بیماری های سیستم عصبی است و با توجه به تظاهرات بالینی آن به میگرن با اورا و میگرن بدون اورا تقسیم می شود. اینکه آیا انواع این بیماری، زیر گروه یک اختلال است و یا باید به عنوان دو ماهیت جداگانه در نظر گرفته شود، مورد بحث می باشد. در این مطالعه پاتوفیزیولوژی و مکانیسم های دخیل در هر یک از انواع بیماری میگرن مورد بحث قرار گرفته است.نتیجه گیریمرور گسترده مطالعات انجام شده حاکی از آن است که مکانیسم های دخیل در پاتوفیزیولوژی میگرن با اورا و بدون اورا از یکدیگر متفاوت است. آشکار شدن تفاوتهای این دو نوع میگرن در این مطالعه از لحاظ اپیدمیولوژی، اتیولوژی و پاتوفیزیولوژی می تواند به درمان بهتر و موثرتر مبتلایان کمک نماید.کلیدواژگان: اختلالات میگرن، سردرد، مغز

Pages 5-8IntroductionJuvenile Myoclonic Epilepsy (JME) is a generalized epileptic syndrome. Age of onset is usually between 12 to 18 years. JME consists of myoclonic jerks, generalized tonic-clonic seizures (GTCs) and typical absence attacks. EEG shows characteristic changes in JME. Long term video-electroencephalography monitoring (VEM) is a helpful diagnostic procedure in the diagnosis of patient with unclear history or EEG findings. In the current study, we aimed to evaluate the role of VEM in diagnosis of refractory epileptic patients.Materials And MethodsThis study is retrospective and descriptive on patients of Epilepsy Monitoring Unit of Razavi Hospital, Mashhad, Iran between March 2011 and March 2012. Telephone interview was scheduled 6-18 months after discharge to evaluate results of VEM on the frequency of seizures, the therapeutic regimes and patients quality of life.Results24 cases with diagnosis of JME were chosen among 250 patients who were admitted with refractory epilepsy. Fourteen of them were female. The average age of patients was 24 years old and the average duration of the seizure attacks was 12.97 years. The mean frequency of GTCs was 2.76 attacks per month and after VEM and proper treatment, it decreased to 0.27 attacks per month.ConclusionVEM is a helpful diagnostic procedure for evaluating of refractory JME epileptic patients.Keywords: Seizures, Epilepsy, Myoclonic Epilepsy, Juvenile, Patients

Pages 9-16IntroductionUnique features of embryonic stem cells (ESCs) such as unlimited proliferation and differentiation into other types of cells make them a favorable tool for biomedical researches as well as a potential source for therapeutic application for neurodegenerative diseases, such as Parkinsons and Alzheimers diseases. In recent years, in vitro methods have been developed which permit the growth of neurons from pluripotent cells in culture. These cells can be maintained as stable, proliferative and undifferentiated cell lines if cultured on feeder layer or in presence of leukemia inhibitory factor. Since ESCs can be proliferated and differentiated, it is possible to generate large numbers of donor cells for neural transplantation. Retinoic acid (RA) is one of the most important morphogenesis, and its embryonic distribution correlates with neural differentiation and positional specification in the developing central nervous system.Materials And MethodsAfter proliferation on the mouse embryonic fibroblast feeder cells in the presence of LIF, for the study of CCE cell line differentiation these cells were cultured to producing cell aggregates (embryoid bodies). The embryoid bodies were under the protocol 4- / 4 (four days in the presence or absence of retinoic acid) at concentration of 10-6 µM retinoic acid for differentiation. Then morphological, molecular and immunocytochemistry examination were used to assess neurological factors.ResultsIn this induction protocol, highly proportion (%80) of ESCs could be induced to differentiation into neuron-like cells. The cells expressed neuroepitelial cell marker nestin. In addition, the results indicated that RA could induce nerve growth factor gene expression in the ESCs.ConclusionThese findings suggest that RA strictly regulates the neuralization and specification during differentiation of mouse ESCs, especially for the differentiation into nerve cells.Keywords: Embryonic Stem Cells, Embryoid Bodies, Nestin, Neurons

Pages 17-21IntroductionInjuries are the most common cause of death among people 1 to 34 years of age and the leading cause of disability. Traffic injuries continue to represent a significant source of injury related mortality and morbidity among young people. Head and neck are prone to traumatic injury during a traffic accident as a result of pendulous movement. This study describes the incidence of head and neck injuries patterns in hospitalized traumatic patients in Tehran, Iran.Materials And MethodsDuring this cross sectional study, all traumatized patients admitted in six main general hospitals in Tehran were enrolled. 2807 head and neck injured patients have been studied thoroughly. Patient's data were collected by trained physicians who were not involved with patient care. They have interviewed patients just for data gathering. To control the data quality, random samples of questionnaires were regularly checked during the study.ResultsOne third of all injured patients had a head or neck injury. There was a male predominance and 75% of patients were 40 years old or younger. Traffic collision was the leading cause of injury followed by fall.ConclusionSafety promotion to prevent traffic collision and fall are crucial to control head and neck injuries in Tehran.Keywords: Head, Neck, Wound, injuries, Epidemiology, Iran

Pages 22-28IntroductionThe term cortical spreading depolarization (CSD) describes a wave of mass neuronal depolarization linked with ionic distribution and cellular swelling. Glutamate release following cell depolarization in extracellular space is a major event in this phenomenon. The hippocampus is widely accepted to play a pivotal role in memory. The aim of the present study was to evaluate the role of glutamate N-methyl-D-aspartate (NMDA) sub receptor on SD-induced memory deficits in rats.Materials And Methods36 juvenile Wistar rat were used to investigate the role of repetitive SD induction on memory performance by T-Maze test. We have investigated the role of MK-801, a NMDA receptor blocker, on the memory retrieval by the same test.ResultsBehavioral assessments showed that memory retrieval significantly impaired following by the repetitive SD induction. However, application of MK-801 improved the memory deficits induced by the repetitive SD.ConclusionOur data suggested that NMDA receptors may play a critical role on protection of memory in SD related disorders.Keywords: Cortical Spreading Depression, Hippocampus, Memory, Brain

Pages 29-33IntroductionPremedication with analgesics prior to surgery leads to reduction in sensitivity and/or blockage of the pain transmission pathway. Gabapentin and Tizanidine are used as premedication drugs. There are limited studies on the effect of Gabapentin as premedication on post-operative pain. Tizanidine compared with Gabapentin has more side effects and a shorter life span. In this study, we compared the analgesic effects of Gabapentin and Tizanidine on pain after elective hysterectomy.Materials And MethodsThis study was performed on 64 patients (30-60 years old) who were referred to Shariati hospital for elective hysterectomy. Patients were randomly divided into two groups (n = 32) Group 1, patients who took 300 mg Gabapentin orally one hour prior to surgery and Group 2, patients who took 8 mg of Tizanidine orally one hour prior to surgery. Pain levels were recorded via Visual Analogue Scales prior to intake of drugs and 12 hours after surgery. Furthermore, morphine intakes in the first 12 hours were compared with the intake at the time of patients first request for morphine.ResultsThere was a significant reduction in the intake dosage of opioid in group 1 in comparison with group 2. The first group requested opioid significantly later than the second group. Both groups had the same level of pain prior to surgery. However, there was significant reduction in pain at 1, 3 and 12 hours after surgery.ConclusionOverall, in comparison with Tizanidine, the use of Gabapentin prior to the surgery decreased pain and the need for opioid during and after the surgery.Keywords: Tizanidine, Gabapentin, Hysterectomy

Pages 34-41IntroductionManipulating the function of neural cells via electrophysiology and pharmacological agents in neuroscience research have long been interesting in unraveling the function of neural circuits, behavior, and attributed disorders. The power of electrophysiology and pharmacologic methods in the manipulation of neural cells varies and therefore researchers have been attempting to enhance their knowledge in this field in recent two decades. Optogenetics has the advantage to take under control both the temporal and spatial resolution in manipulating desired neural cells. In optogenetics, control over the function of neural cells is possible through shining the light onto the cells that bear light sensitive molecules.ConclusionOptogenetics come to help the researchers to know how the neural cells, circuits, and systems work and there is an attempt to move toward the treatment of nervous system disorders by this technology. Pages 42-49IntroductionTransient receptor potential vanilloid 1 (TRPV1) belongs to a family of ligand gated ion channels. This non-selective cation channel is permeable to Na, K and highly to Ca2 ions. It acts as a trigger for Ca2 mediated cell signaling. Although this channel has been previously found highly expressed in dorsal root ganglion, there is a line of evidence indicating the remarkable expression of TRPV1 channels in other parts of the central nervous system.ConclusionThere is evidence to suggest that TRPV1 channels in the brain contribute in many basic neuronal functions, including neurotransmitter release and synaptic plasticity.Keywords: TRPV1 receptor, Synaptic Transmission, Neuronal Plasticity

Pages 50-54IntroductionMigraine is one of the most common neurological disorders. It has been classified into migraine with aura (MA) and migraine without aura (MO) by defining two separate diagnostic criteria. This classification has been subject to many debates over the years about whether MO and MA are subtypes of the same disorder or should be considered as two separate entities. This study has reviewed the different mechanisms involved in the pathophysiology of the MO and MA.ConclusionSeveral studies have shown that the mechanisms involved in the two forms of migraine (MO and MA) differ from each other. Exclusion of mechanisms involved in the pathophysiology of these diseases may help to invent the more effective treatment.Keywords: Migraine Disorders, Headache, Brain
